Attach following documents with your form

• Photocopy of CNIC/Bay Form of the candidate.




• Photocopy of CNIC/Bay Form of the father/guardian.




• Three Photocopies of Matriculation /Intermediate certificate.




• Character certificate from the institution last attended.




• Three recent passport sized photographs. One to be pasted on application form and two be provided at the time of admission.




• Photocopy of NOC for those candidate who have passed their last exam from other than Lahore Board. The original NOC should be submitted at the time of admission.
